Visiting a new doctor can be anxiety provoking with thoughts of "What will he or she do?" and "How long will I wait?"
Please come 15 minutes early
Our goal is to make all our patients as comfortable as possible. In order to help us do this, we ask that you come 15 minutes early to your appointment to fill out our New Patient Questionnaire.
You will fill this form out on a computer, but we have attached the form for downloading so you can prepare your answers before your visit. If you do not feel comfortable using a computer, you are welcome to fill the form out by hand.
Please bring your previous records
If you have been referred by a different physician, or if you have previous records, including CT scans, MRI scans, EMG studies or X-rays, we kindly ask you to bring those along as well, if possible, so we can develop a full case history.
